Medications delivered,
managed, and coordinated.

Ohana coordinates access to licensed pharmacy partners who provide home delivery, medication synchronization, and adherence support, making medication management simpler for seniors and the families who support them.

What's Available

Pharmacy services, coordinated through licensed partners.

From simple home delivery to complex compounding needs, Ohana connects patients with the pharmacy services that fit their clinical situation.

Home Delivery

Prescription medications delivered directly to the patient's home. No trips to the pharmacy required. Especially valuable for homebound patients and those with mobility limitations.

Medication Sync

Synchronizing multiple prescriptions to a single refill date, reducing the number of pharmacy trips and minimizing the risk of gaps in medication coverage.

Adherence Support

Blister packaging, bubble packs, and medication management tools to help patients with complex regimens stay on schedule and reduce medication errors.

Compounding

Specialty compounding pharmacy services where available, for patients who require customized medication formulations, alternative dosage forms, or flavor-modified preparations.

How It Works

Getting started is simple.

Ohana coordinates the transition to a home delivery pharmacy partner, handling the introduction and setup so the patient and family can focus on care, not logistics.

1
Physician Prescriptions in Place

Pharmacy services require valid prescriptions from the patient's treating physician or authorized prescriber. Existing prescriptions can typically be transferred to the partner pharmacy.

2
Coordination Request

Contact Ohana to coordinate pharmacy access. We facilitate the introduction to the partner pharmacy and help confirm coverage and formulary eligibility.

3
Partner Pharmacy Setup

The partner pharmacy manages prescription transfer, insurance verification, delivery scheduling, and billing directly with the patient, family, and prescriber.

4
Ongoing Service

Refills, delivery adjustments, and billing questions are managed directly by the partner pharmacy. Ohana remains available if coordination support is needed.

Who can benefit from pharmacy coordination?

  • Seniors managing multiple prescriptions who struggle with pharmacy access
  • Homebound patients who cannot safely travel to a pharmacy
  • Home health and hospice patients requiring ongoing medication delivery
  • Patients with complex regimens needing blister packaging support
  • Patients requiring specialty compounded formulations
Pharmacy services are regulated by the California Board of Pharmacy. Ohana is not a pharmacy or pharmacy benefit manager. All drug dispensing and prescription fulfillment is performed by licensed partner pharmacies. Insurance coverage is determined by the partner pharmacy and the patient's plan.
Coverage

Insurance & accepted plans.

Coverage, copayments, and formulary eligibility are managed by the partner pharmacy and the patient's insurance plan, not by Ohana.

Many Medicare Part D plans, Medi-Cal Rx, and private insurance plans may be accepted by our pharmacy partners, depending on the partner pharmacy and plan participation. Contact our coordination team to confirm eligibility with the specific partner pharmacy that best fits your needs.

Commonly accepted coverage types

  • Medicare Part D prescription drug plans
  • Medi-Cal Rx (California Medicaid)
  • Private / commercial insurance plans
  • Medicare Advantage prescription benefits
  • Self-pay options available

Coverage varies by plan and formulary. The partner pharmacy confirms eligibility directly with the patient's insurance at the time of setup.
