Pressure Injuries
Stages 1 through 4 pressure ulcers and deep tissue injuries, including prevention protocols, offloading strategies, wound bed preparation, and dressing selection. Pressure injury management requires consistent specialist oversight to prevent deterioration and support closure.
Surgical Wounds
Post-operative wound monitoring, dehiscence management, and staple or suture care following discharge. Surgical wounds that require skilled assessment and dressing changes beyond what a caregiver can provide are appropriate for specialist coordination through our network.
Diabetic Ulcers
Neuropathic and ischemic diabetic foot ulcers requiring specialized management, infection monitoring, and offloading. Diabetic ulcers carry significant risk for complications; early and consistent specialist involvement is critical to preventing escalation.

Clinical Intake
Ohana gathers wound history, current treatment status, physician orders, and relevant medical context to accurately characterize the patient's needs. This information is used to match the patient with an appropriate licensed specialist from our network, not to make clinical assessments ourselves.
Partner Specialist Matching
We coordinate with a licensed wound care clinician from our partner network to schedule an initial in-home assessment. Specialist selection is based on wound type, patient location, and clinical availability, ensuring the right expertise reaches the patient.
Ongoing Coordination
Ohana facilitates ongoing communication between the wound care specialist, the patient's home health agency, and the treating physician throughout the course of treatment. We serve as a coordination layer, keeping all parties aligned without inserting ourselves into clinical decisions.
