Senior Living Options

Care levels we navigate

Every senior's needs are different. We're fluent in all residential care settings, from independent living for active adults to skilled nursing for those requiring continuous medical oversight.

Board & Care

Board and care homes are smaller residential settings that may support seniors who need help with daily routines in a more home-like environment. Typically serving 2–6 residents, these homes offer personal care, meals, and 24-hour supervision in a shared household setting.

Often a fit for

  • Seniors who need daily personal care assistance
  • Those who prefer a smaller, more intimate setting over a large facility
  • Individuals who don't require 24/7 skilled nursing oversight

Ohana helps families compare board and care options and coordinates introductions to vetted homes across Southern California. Availability, pricing, and eligibility vary by home and provider.

Assisted Living

Assisted living communities may support seniors who need help with daily activities while maintaining as much independence as possible. Residents typically have private or semi-private apartments with access to staff, meals, social activities, and on-site amenities.

Often a fit for

  • Seniors who need help with bathing, dressing, or medication management
  • Those who benefit from social programming and on-site amenities
  • Family caregivers approaching or at a transition point

Ohana helps families compare assisted living options and coordinates introductions and next steps with communities in our network. Availability, pricing, and eligibility vary by community.

Independent Living

Independent living communities are designed for active older adults who want a maintenance-free lifestyle, social connection, and convenient amenities, without the responsibilities of home ownership. Meals, housekeeping, transportation, and activities are typically included.

Often a fit for

  • Active seniors who are largely self-sufficient
  • Those seeking community, activities, and daily convenience
  • Adults who want to simplify their lives without sacrificing independence

Ohana helps families identify independent living communities that match lifestyle preferences, budget, and location. We coordinate introductions and guided tours at no cost to families.

Nursing Home / Skilled Nursing

Skilled nursing facilities (SNFs) support individuals who may need a higher level of medical oversight, rehabilitation services, or around-the-clock licensed nursing care. They are licensed medical facilities staffed by nurses and therapists, and are often the appropriate setting following an acute hospital stay.

Often a fit for

  • Post-acute patients transitioning from hospital discharge
  • Individuals requiring complex wound care, IV therapy, or rehabilitation
  • Seniors with chronic conditions requiring licensed clinical monitoring

Ohana coordinates referral and intake support for skilled nursing placement, with established contacts at facilities across Southern California. Availability and coverage vary by facility, eligibility, and payer.

Home Care

Home care may support seniors with non-medical daily assistance, including companionship, meal preparation, personal care, and routine support, in the comfort of their own home. Home care aides are not licensed clinical staff and do not perform skilled medical procedures.

Often a fit for

  • Seniors who prefer to remain at home with reliable support
  • Those who need companionship, light housekeeping, or personal care
  • Families managing caregiving responsibilities from a distance

Ohana coordinates access to vetted home care providers through our network. Availability may vary by service area, caregiver availability, and scheduling. Provider services are delivered independently.

Memory Care

Memory care communities are designed for seniors living with Alzheimer's, dementia, or memory-related conditions who may benefit from structured support and safety-focused environments. These specialized communities typically feature secured perimeters, trained staff, and therapeutic programming.

Often a fit for

  • Seniors with diagnosed Alzheimer's disease or other forms of dementia
  • Those who require a structured, safety-focused daily environment
  • Individuals whose safety cannot be adequately maintained in other settings

Ohana helps families understand their memory care options and navigate the intake process with compassion and clarity. We coordinate introductions to communities that specialize in cognitive support.

Continuing Care Retirement Community

Continuing care retirement communities (CCRCs) offer multiple levels of senior living on one campus, typically including independent living, assisted living, and skilled nursing, allowing residents to transition between care levels as needs change without moving to a new community.

Often a fit for

  • Seniors planning ahead who want continuity of community as care needs evolve
  • Couples with differing care levels who want to remain on the same campus
  • Those who value long-term care relationships and predictable transitions

Ohana helps families evaluate CCRC options including contract types, fee structures, and services included at each care tier. We coordinate introductions and guided visits at no cost to families.

How It Works

Our placement process

We don't send you a spreadsheet and wish you luck. From initial conversation to move-in day, Ohana's placement advisors are present, reachable, and working on your behalf at every stage.

Begin the Process
Free Consultation

We listen to your situation, your loved one's care needs, and your family's priorities, with no agenda other than understanding the full picture. There's no commitment required and no fee for this initial conversation.

Needs Assessment

We evaluate current health status, mobility, cognitive capacity, activities of daily living, behavioral considerations, and personal preferences. This assessment shapes every recommendation we make.

Options Presented

We identify 3–5 facilities that match your loved one's care needs, your family's budget, and your preferred geographic area. We explain each option plainly: the strengths, the limitations, and why it made the list.

Guided Tours

We coordinate visits to each facility and accompany you when requested. We know the right questions to ask about staffing ratios, inspection history, medication management practices, and how facilities handle care transitions.

Transition Support

Our involvement doesn't end at move-in. We stay engaged through the first week, helping navigate admission paperwork, coordinate any additional services your loved one needs, and address concerns that arise during the adjustment period.

Common Questions

Frequently asked questions

Placement decisions come with a lot of questions. These are the ones families ask us most often. If yours isn't here, call us; there's always someone available to talk.

(909) 206-4228

How long does placement typically take?

Most families identify a suitable facility within 5–10 business days. Urgent placements, following hospital discharge or a care crisis, can often be completed within 24–48 hours. Timeline depends on the specificity of the family's requirements and current availability at preferred facilities.

Can Ohana help with emergency placements?

Yes. If your loved one is being discharged from the hospital and needs immediate placement, contact us directly. We prioritize urgent cases and have established relationships with facilities across Southern California that can accommodate short-notice admissions.

What if we don't like any of the options presented?

We go back to the search. There's no pressure, no deadline, and no obligation. Our goal is the right fit, not a fast placement. If the initial options don't resonate, we refine our criteria and identify alternatives.

Does Ohana work with all types of facilities?

We coordinate with a network of vetted facilities across Los Angeles, Orange, Riverside, and San Bernardino counties, spanning independent living, assisted living, board and care homes, memory care communities, and skilled nursing facilities. If your preferred location is outside our current network, we will tell you directly rather than send you somewhere we cannot vouch for.
