Ohana Senior Care Agency is a healthcare management organization coordinating access to the full continuum of home-based and community care across Southern California. We exist to make the complicated simple and the overwhelming manageable.

For too many families, navigating senior care means making dozens of phone calls, deciphering insurance requirements, and hoping the right provider shows up. It shouldn't be that hard.
Ohana was built on a simple premise: when care coordination actually works: when someone takes ownership of the intake, the routing, the communication, and the follow-through, patients get better outcomes and families get their peace of mind back.

Ohana operates two types of services. Understanding this distinction is important for families choosing care, and for providers partnering with us.
Services we operate in-house.
For our Ohana-Direct services, Ohana owns, staffs, and operates the service. When you call about Senior Placement or Medical Transportation, you are working with Ohana's own team from first contact to service completion.

For clinical and specialized services, Ohana manages intake, matching, communication, and follow-through, while clinical care is delivered by independent licensed providers within our credentialed partner network. We stay involved. We don't just hand off.
