All Services

One coordination point.
A full spectrum of care.

Ohana coordinates access to 13 home-based and community care services across Southern California, from medical transportation and senior placement to home health, hospice, and specialized diagnostics.

How Our Services Work

Two service models. Full transparency.

Ohana operates two types of services: those we run directly, and those we coordinate through our network of independently licensed providers. Every card in our service grid is labeled clearly.

Ohana-Direct
Operated directly by Ohana.

Ohana owns, operates, and staffs these services in-house. When you request an Ohana-Direct service, you are working with Ohana's own team, not a third-party provider.

Senior Placement Medical Transportation (NEMT)
Network-Coordinated
Coordinated through licensed partners.

Clinical services are provided by independent licensed agencies and practitioners within the Ohana network. Ohana manages coordination, including intake, matching, communication, and follow-through, but clinical care is delivered by the partner.

Clinical services and network-coordinated services on this page are provided by independent licensed providers within the Ohana network. Ohana Senior Care Agency does not directly render clinical care except where explicitly noted. Eligibility, clinical decisions, and care delivery are the responsibility of the treating licensed provider.

Coverage note: Insurance, Medicare, HMO, and other coverage options vary by service, provider, plan, eligibility, and authorization requirements. Ohana helps coordinate access to appropriate service partners but does not guarantee coverage, benefits, authorization, or payment.
