Ohana coordinates access to 13 home-based and community care services across Southern California, from medical transportation and senior placement to home health, hospice, and specialized diagnostics.
Two service models. Full transparency.
Ohana operates two types of services: those we run directly, and those we coordinate through our network of independently licensed providers. Every card in our service grid is labeled clearly.
Ohana owns, operates, and staffs these services in-house. When you request an Ohana-Direct service, you are working with Ohana's own team, not a third-party provider.
Clinical services are provided by independent licensed agencies and practitioners within the Ohana network. Ohana manages coordination, including intake, matching, communication, and follow-through, but clinical care is delivered by the partner.
Coverage note: Insurance, Medicare, HMO, and other coverage options vary by service, provider, plan, eligibility, and authorization requirements. Ohana helps coordinate access to appropriate service partners but does not guarantee coverage, benefits, authorization, or payment.

Senior Placement
Consultative advisory service helping families find the right senior living option (independent living, assisted living, memory care, or skilled nursing) without sales pressure.
Non-Emergency Medical Transportation
Ambulatory, wheelchair, and gurney transport operated by Ohana's own fleet. Serving hospitals, dialysis centers, SNFs, and home-to-appointment across Southern California.
Clinical services through trusted licensed partners.
Ohana manages coordination, intake, and follow-through. Clinical care is delivered by independent licensed providers in our credentialed network.
Meal Program
Coordinated access to medically tailored meal delivery: cardiac, renal, diabetic, dysphagia, and general senior diets, through a licensed partner program.
Caregiving
Non-medical in-home caregivers matched through Ohana's coordination process, assisting with daily activities, companionship, and household support.
Home Health
Access to skilled home health services (RN/LVN visits, physical therapy, occupational therapy) through Medicare-certified agencies in our network.
Wound Care
In-home wound care coordination with licensed clinical specialists for pressure injuries, surgical wounds, diabetic ulcers, and venous/arterial wounds.
Palliative & Hospice
Coordination support for palliative care and end-of-life services through trusted hospice partner agencies, with family navigation support throughout.
Mobile Doctor (PCP)
House-call primary care coordination through board-certified physician partners, for homebound patients, post-surgical recovery, and chronic disease management.
Mobile Lab & Diagnostics
At-home specimen collection and diagnostic services through CLIA-certified partner laboratories: PCR, blood panels, wound culture, respiratory, and more.
Portable X-Ray
Portable in-home imaging (chest X-ray, extremities, and musculoskeletal) brought to SNFs, assisted living facilities, and private residences on physician order.
Pharmacy
Coordinated access to home delivery pharmacy services (medication sync, adherence support, and compounding where available) through licensed pharmacy partners.
Mental Health
Access to geriatric-aware behavioral health and counseling services through independently licensed mental health clinicians, supporting seniors and their families.
Staffing
Pre-screened clinical and caregiving staff coordinated through vetted staffing partners: RNs, LVNs, CNAs, and caregivers for facilities with supplemental staffing needs.
