Skilled services available through our network
Home health agencies in the Ohana network are Medicare-certified and provide a full range of skilled services in the patient's home. These services are provided by independent licensed clinicians, not by Ohana directly.
Skilled Nursing
RN and LVN home visits for wound assessment and care, medication management and reconciliation, IV therapy, catheter care, and chronic disease monitoring. Skilled nursing visits are ordered by a physician and conducted by licensed nurses through our partner agencies.
Physical & Occupational Therapy
In-home PT and OT for post-surgical recovery, stroke rehabilitation, fall prevention, strength restoration, and functional independence. Therapists work within the patient's home environment, addressing real-world barriers to safe mobility and daily activity.
Speech Therapy & Social Work
Speech-language pathology for dysphagia evaluation and treatment, cognitive communication, and post-stroke language recovery. Medical social work services support discharge planning, community resource connection, and care coordination for complex patient situations.
Who home health coordination serves
Home health is most appropriate for patients who are homebound or transitioning out of a facility. Ohana can coordinate access for several patient situations.
Post-Discharge Patients
Patients leaving hospitals or skilled nursing facilities who need continued skilled care at home. Post-acute home health helps bridge the gap between inpatient care and independent recovery, reducing readmission risk and supporting a safe transition.
Homebound Seniors
Patients whose medical condition makes leaving home difficult or medically inadvisable may be eligible for Medicare-covered home health services. Homebound status is determined by the treating physician and the home health agency. Ohana facilitates the coordination and referral process.
Chronic Disease Management
Patients managing diabetes, COPD, congestive heart failure, or other complex conditions who benefit from regular skilled nursing oversight at home. Episodic skilled nursing visits can catch early deterioration and reduce unnecessary emergency utilization.
Medicare & Medi-Cal coverage
Home health services may be covered under Medicare Part A or Part B when ordered by a physician and provided by a Medicare-certified agency. To qualify, the patient generally must:
- Be under the care of a physician who orders the home health services
- Be considered homebound as defined under Medicare criteria
- Require skilled nursing, physical therapy, speech therapy, or occupational therapy
- Receive services from a Medicare-certified home health agency
Medi-Cal may also cover home health services for eligible beneficiaries. Coverage determinations are made by the treating physician and the home health agency, not by Ohana.
Coverage & billing: Ohana does not bill Medicare or Medi-Cal. Coverage eligibility, prior authorization, and billing are managed entirely by the independent home health agency providing services. Ohana's role is coordination and referral only. Patients and families should confirm coverage eligibility directly with the home health agency and their insurer.
Home health services coordinated through Ohana are provided by independent Medicare-certified agencies within our network. Ohana does not directly render skilled nursing, physical therapy, occupational therapy, speech therapy, or any other clinical care. All clinical decisions, including plan of care, visit frequency, and treatment approach, remain with the treating agency and its licensed clinicians.
