Meals that respect the appetite and the medical chart.

Ohana coordinates access to medically tailored meal delivery for seniors who have specific dietary needs. Whether post-discharge nutrition support or ongoing meal service, we connect you with a licensed partner program that gets it right.

Diet types available through our partner program

Every meal plan is matched to the patient's medical requirements, not a generic senior menu. The following medically tailored diet categories are available through our licensed partner program.

Cardiac Diet

Sodium-restricted meals designed to support heart health and blood pressure management. Appropriate for patients with hypertension, heart failure, or post-cardiac-event recovery who require controlled sodium intake.

Renal Diet

Carefully controlled potassium, phosphorus, and sodium for patients managing chronic kidney disease or on dialysis. Renal diets require precise formulation; our partner program works from physician-specified parameters.

Diabetic Diet

Carbohydrate-managed meals to support blood glucose stability for patients with Type 1, Type 2, or diet-controlled diabetes. Portion-appropriate and consistent across meals to reduce glycemic variability.

Dysphagia

Modified-texture meals for patients with swallowing difficulties, available in pureed and minced formats following IDDSI standards. Appropriate for patients post-stroke, with neurological conditions, or who have been assessed for aspiration risk.

General Senior

Nutrient-dense, age-appropriate meals for seniors without specific medical dietary restrictions. Balanced macronutrient profiles, appropriate portion sizes, and foods selected to support healthy aging and consistent nutritional intake.

How coordination works

Ohana handles the intake and handoff, so families don't have to navigate a partner program on their own. Here's what to expect from start to service.

Intake Request

Contact Ohana to start the meal program coordination process. We gather dietary needs, medical history, current diagnoses, and delivery preferences, including schedule, location, and any physician-specified parameters.

Diet Assessment

We review the patient's needs with our licensed partner to confirm the appropriate meal plan category. Where physician orders or dietitian notes are available, these are incorporated into the plan selection process.

Delivery Setup

The partner program establishes a delivery schedule, confirms all dietary requirements directly with the patient or family, and sets up service. Delivery frequency and portion quantities are confirmed before the first delivery.

Ongoing Coordination

Ohana remains your point of contact for any changes, concerns, or adjustments to the meal plan. If a patient's condition changes or their physician updates dietary orders, we facilitate communication with the partner to update the plan accordingly.

Important: Meals are provided through a licensed partner program coordinated by Ohana. Ohana does not prepare, package, or deliver meals directly. Dietary suitability should be confirmed with the patient's treating physician before initiating any medically tailored meal plan. Ohana's role is coordination only.
