Coordination works because
of the partners behind it.
Ohana's ability to coordinate access to 13 services rests on a network of independently licensed providers who share our standards for credentialing, communication, and patient-centered care. These are the organizations we trust with the families who call us.
Independent partners. Shared standards.
Every provider in the Ohana network is independently licensed, operated, and accountable for their own clinical and service delivery. Ohana manages coordination, including intake, routing, communication, and follow-through, but clinical care remains with the licensed provider.
This distinction matters. Ohana is a healthcare management organization, not a staffing firm, a referral directory, or a clinical provider. Our partners are chosen because of their credentials, service quality, and commitment to patient-centered care, not for commercial relationships.
What we verify before any referral
- Active California state license (CDPH, BRN, BVNPT, Board of Pharmacy, etc.)
- Medicare/Medi-Cal certification where applicable
- Liability insurance coverage maintained and current
- No active CDPH or enforcement actions at time of onboarding
- Service area confirmation for patient location

How Ohana works with its partners.
Our coordination model is designed to keep things clean: clear roles, clear responsibilities, and no ambiguity about who does what.
When a family or referral partner contacts Ohana, our coordination team handles intake, assessing the patient's needs and identifying the right partner for their location and situation.
Ohana contacts the appropriate network partner, confirms availability and capacity, and facilitates the introduction between the patient/family and the provider.
During the service engagement, Ohana remains available as the communication bridge between family, provider, and any other parties involved in the patient's care plan.
Ohana follows up with both the family and the partner to confirm service was delivered, address any open issues, and identify if additional coordination is needed.
What we expect from network partners
- Active California license at all times
- Responsive communication with Ohana coordination team
- Timely confirmation of service capacity and scheduling
- Prompt notification of any patient safety concerns
- Patient-first, documentation-first operations
