What In-Home Visits Include

Partner physicians bring the clinical capabilities of a primary care office directly to the patient.

  • Preventive care, health screenings, and annual wellness assessments
  • Chronic disease management and medication review
  • Acute illness evaluation and treatment
  • Physician referrals to specialists and ancillary services
  • Care coordination with other treating providers
  • Medicare Annual Wellness Visit (eligibility determined by physician practice)

Mobile primary care coordination network.

Ohana coordinates with 101 Mobile Medical Group and other board-certified physician partners serving Southern California. Partner physicians maintain independent medical practices and are responsible for all clinical decisions.

Partner profiles are available on the Partners page.

Medicare and private insurance.

House-call primary care may be covered under Medicare and private insurance plans. Coverage eligibility, copayments, and prior authorization requirements are determined by the physician practice and the patient's insurance plan, not by Ohana.

Ohana does not bill for physician services and does not determine Medicare eligibility. All clinical decisions, billing, and care responsibility rest with the independent physician practice.

Common Questions

Frequently asked questions

Questions families and facilities ask about mobile primary care coordination through Ohana.

A mobile primary care physician provides house-call medical services at the patient’s location — at home, in assisted living, board and care, or skilled nursing settings — rather than in a traditional clinic. This model is particularly valuable for homebound or mobility-limited patients.

No. Ohana coordinates access to independently practicing physicians and physician groups in our mobile PCP network. The physicians are not employees of Ohana. Clinical decisions and medical management remain with the treating physician.

Mobile primary care visits may be covered under Medicare, Medicare Advantage, Medi-Cal, or other plans depending on eligibility and plan terms. Coverage, prior authorization, and billing are handled by the mobile physician practice — not Ohana.

Mobile primary care may include physician assessment, chronic disease management, medication management, wound evaluation, lab ordering, specialist referrals, and care plan coordination. Available services depend on the treating physician and the patient’s clinical needs.
